Partners for Community Health Hyannis eventEarlier this week we visited Harbor Health Services in Hyannis to announce $3 million in grants to health centers on Cape Cod, the Islands and partnering community health centers (CHCs).

In our two years partnering with Neighborhood Health Plan and the Massachusetts League of Community Health Centers we’ve awarded over $10 million to Massachusetts health centers. The projects awarded will allow the CHCs to transform operations, patient communications and service delivery while collaborating with other health centers to find new solutions to common problems.

For example, Harbor Community Health Center in Hyannis will partner with 10 other CHCs on a project focusing on electronic health records. The centers will work together on large-scale technology projects and prepare to join Mass HIway, the state’s electronic health information exchange – all of which should lead to more coordinated care and more streamlined public health reporting, at a lower cost.

“This health center is an example of innovation and excellence. This is Massachusetts health care,” noted Partners CEO and President Dr. Gary Gottlieb. “These grants are an investment in the future of health care delivery and health care policy.”

Another collaborative grant awarded to the Cape Cod region will allow six health centers to improve the triage of after-hours calls – thereby eliminating unnecessary trips to the ER, and improving patient satisfaction and care.

In this round, 35 CHCs received grants to support their continued efforts to improve access, promote health equity and provide care for patients in their communities.
